This was posted over at theonering.com... I think I've seen it before, when it was said to be the the last pages of the theatrical version script.

SAM
Well, I’m back.
PULL BACK: Sam, Rosie and Elanor slowly walk back inside BAG END. Hobbit Children play in nearby fields as the setting sun sets in the western sky ablaze with deep purple hues. The FADING LIGHT casts soft shadows in the idyllic, green land of the SHIRE.
GALADRIEL V/O
Thus ended the Third Age of Middle Earth.
IMAGE: Still pulling back from the Shire we see a dreamy, ghostly image of an ancient Sam, Merry and Pippin in the GREEN DRAGON. Merry and Pippin are SINGING loudly. Sam is smiling.
GALADRIEL V/O
History became legend… Legend became myth, and some things that should not have been forgotten… Were lost.
IMAGE: An ancient Sam standing atop the TOWER HILLS looking towards the harbours of the GREY HAVENS. He turns and kisses Elanor on the cheek, handing her a RED BOOK. He walks off towards the SEA
GALADRIEL V/O
But not all things.
IMAGE: A youthful Legolas and Gimli EXPLORING Fangorn Forest.
GALADRIEL V/O
Hope is eternal, and friendship withstands even the darkest days.
IMAGE: A mesmerized Gimli and Legolas staring in wonder at the CAVES OF HELM.
GALADRIEL V/O
The Power of Terror and Hate, sowed in the hearts of Men is a seed that does not die and can never be destroyed; and ever and anon it sprouts anew, and will bear dark fruit even unto the latest days.
IMAGE: Arwen holding Aragorn’s hand on his DEATHBED.
GALADRIEL V/O
But the Shadow cannot conquer for ever.
IMAGE: Aragorn, peaceful in death, rests in the TOMB OF KINGS. On either side of him are Merry and Pippin on their tombs, dressed in the armour of ROHAN and GONDOR.
IMAGE: Queen Arwen, still YOUNG, laying on a bed in a long abandoned RIVENDELL. She closes her eyes, and dies.
GALADRIEL V/O
For in the end it is only a small and passing thing. There is light and high beauty for ever beyond its reach.
IMAGE: Legolas and Gimli on an ELVEN SHIP, sailing into the west …. Leaving the shores of Middle-earth behind. On the Horizon there is a veil of silver rain, which draws aside to reveal the beautiful land of VALINOR.

Even if it isn't the real thing, it's amazing!
edit... ah, looks like it was just an April Fool's joke over at theonering.com from last year. Ah well, I still think it's great!
